This is the Official ITN On Court Assessment Score Sheet that was developed by the ITF. All Official ITN On Court Assessments must be conducted using this official Score Sheet.

The Player/Parent/Guardian consent form is attached to the score sheet to enable the assessment results to be entered on to the official website at www.oncourtassessment.com

All participants must complete the consent form prior to the assessor entering the assessment results and player details on the website.
