If they already have a PPN please do not create a new Player Profile for them. This is a picture of the Create New Player Profile page. Enter the player details as shown on the Assessment Score Sheet. Please ensure that you enter the player's correct Date of Birth to ensure that the player's score will appear in the correct age category and click on the Submit button.
